Staten Island Care Center is seeking an experienced and compassionate Social Services Worker to support our residents and families. The ideal candidate will have strong experience in long-term care, excellent communication skills, and a passion for resident advocacy and quality care.
Responsibilities
- Provide social, emotional, and psychosocial support to residents and families.
- Complete assessments, social histories, progress notes, and care plans.
- Participate in care plan meetings, MDS, discharge planning, and interdisciplinary team meetings.
- Coordinate discharges, transportation, referrals, and community resources.
- Assist residents and families with Medicare, Medicaid, financial assistance, and community services.
- Review and coordinate MOLST/POLST and advance directive documentation.
- Advocate for resident rights, preferences, and quality of life.
- Maintain accurate documentation and ensure compliance with applicable regulations.
- Collaborate closely with nursing, medical, rehabilitation, and other departments.
- Participate in quality assurance activities, facility surveys, and departmental initiatives.
- Build strong relationships with residents, families, staff, physicians, and community agencies.
- 1-2 years of social services experience required.
- Bachelor's or master's Degree required.
- Long-term care, skilled nursing, rehabilitation, or healthcare experience preferred.
- Knowledge of care planning, discharge planning, MDS, Medicare/Medicaid, and community resources.
- Strong communication, organization, and documentation skills.
- Compassionate, professional, dependable, and highly organized.
- Strong problem-solving skills and ability to work effectively as part of an interdisciplinary team.
- Experience with EMR systems preferred.
