Position Overview
Nassau Rehabilitation and Nursing Center is seeking a detail-oriented and compassionate Admissions Coordinator to join our team. This role is essential in supporting the admissions process, ensuring smooth transitions for residents and families, and collaborating with interdisciplinary teams to maintain census growth and compliance.
Responsibilities
- Provide coverage for the Admissions Department in the absence of the Admissions Director.
- Support the Admissions Director with all department functions.
- Attend Morning Report as needed by the Director or Administrator.
- Complete and distribute daily AM census and End-of-Day (EOD) reports.
- Share all admissions-related updates with the interdisciplinary team (alerts, chats, lists).
- Prepare and finalize Admissions Agreements for new residents.
- Conduct regular audits to ensure 100% compliance with PRI, screens, agreements, and logs.
- Manage referrals (Allscripts, Navi, faxes) with a 90% response rate within 15-30 minutes.
- Enter and upload data into Sigma and billing systems prior to each new admission.
- Partner with marketing/outreach teams and hospitals to maximize admissions opportunities.
- Communicate with families to assist in referral conversion and highlight facility strengths.
- Stay informed on facility capabilities, policies, and marketing features.
- Demonstrate strong knowledge of insurance, payor sources, disenrollments, and SNF finances.
- Answer calls and assist with insurance or general inquiries.
- Maintain the daily hospitalization list and share updates with outreach and social work.
- Prepare for new admissions, including charts, room arrangements, and equipment coordination.
- Collaborate with nursing, housekeeping, and concierge to ensure readiness of rooms.
- Assist outreach team with marketing materials, giveaways, and promo item inventory.
- Send out admission alerts via email.
- Collect financial and intake information during the admissions process.
- Provide family tours and attend meetings when necessary or in the Director’s absence.
- Report departmental concerns or observations to the Admissions Director.
- Perform general administrative tasks as assigned.
- Be available outside standard business hours if needed.
- Minimum 1 year of Admissions Coordinator experience (required).
- Knowledge of Medicare requirements, PDPM, HMO rules, and prescription plans.
- Familiarity with EMR software systems.
- Understanding of CMS guidelines, DOH regulations, and insurance policies.
- Strong verbal and written communication skills.
- Established relationships with local hospitals (preferred).
