Job Responsibilities
The Program Manager/Designated Manager is able to manage programs and provide oversight of the services provided under the 245D license. Substantial experience supervising a program providing direct support services. A person who is positive, a team player and willing to learn.
- Manage a caseload of clients, ensuring service quality, client safety, and satisfaction.
- Coordinate and conduct all required client meetings (45-day, 60-day, Semi-Annual, Annual)
- Develop, implement, and review Individualized Service Plans (ISP) and Individual Abuse Prevention Plans (IAPP) every 6 months, sharing documents with the care team.
- Manage incident reporting and investigations for internal and external cases of suspected abuse or neglect.
- Maintain accurate, up-to-date client records in compliance with 245-D and HIPAA regulations.
- Develop training materials and manage the staff training portal to ensure timely completion of DHS-required training.
- Lead monthly staff meetings and communicate expectations clearly to ensure consistent service delivery.
- Serve as liaison between clients, guardians, case managers, and external providers.
- Conduct onboarding and orientation for new DSPs in collaboration with HR, including 30-, 60-, 90-day, and annual reviews.
- Review all DSP progress notes and provide timely, corrective feedback to maintain documentation standards.
- Manage incoming calls and text messages related to staffing, client concerns, and scheduling
- Provide direct care and supervision to clients as needed; participate in on call rotation
Required Qualifications
- Bachelor's degree in a human services or related field, preferred
- 3 years of experience supervising programs in the human service field
- Must qualify as a Program Manager /Designated Manager per MN DHS qualifications
- Valid driver's license and reliable transportation with valid auto insurance
- Demonstrated experience in 245D compliance
