Job Summary
The Program Manager / Designated Coordinator is responsible for overseeing and ensuring the delivery of high-quality, person-centered services to individuals served by Dynamic Relocation Services. This role provides leadership, training, and supervision to Direct Support Professionals (DSPs), monitors service delivery and progress toward outcomes, and ensures compliance with Minnesota 245D regulations. The Program Manager/Designated Coordinator also evaluates service effectiveness, facilitates staff development, and provides hands-on support to individuals when DSP coverage is needed. This is a full-time, Monday–Friday, 9:00 AM–5:00 PM position.
Responsibilities
- Supervise, train, coach, and evaluate DSP staff, including providing instruction, assistance, and direct observation.
- Maintain staffing schedules according to program needs, safety guidelines, and budget.
- Oversee service delivery to ensure individuals’ service plans are followed and outcomes are achieved.
- Lead actions to facilitate accomplishment of outcomes for individuals served.
- Conduct audits and compliance checks to ensure quality services.
- Evaluate service effectiveness and monitor progress on individual outcomes.
- Communicate effectively with staff, individuals, guardians, and case managers.
- Respond to emergencies and provide hands-on support to individuals as needed if DSP coverage is required.
Qualifications
- To serve as a Designated Coordinator, candidates must meet at least one of the following education and work experience combinations:
- Bachelor’s Degree in human services, education, or health AND 1 year of full-time experience providing direct care to persons with disabilities or adults age 65 OR equivalent work experience providing care or education to vulnerable adults or children.
- Associate Degree in human services, education, or health AND 2 years of full-time experience providing direct care to persons with disabilities or adults age 65 OR equivalent work experience providing care or education to vulnerable adults or children.
- Diploma AND 3 years of full-time experience providing direct care to persons with disabilities or adults age 65 OR equivalent work experience providing care or education to vulnerable adults or children.
- Training Experience (Alternative Route). At least 50 hours of education/training related to human services and disabilities AND 4 years of full-time experience providing direct care to persons with disabilities or adults age 65 , under supervision of someone who meets one of the above education/experience tracks.
Additional Qualifications:
- Ability to maintain Monday–Friday, 9AM–5PM schedule.
- Strong organizational, problem-solving, and communication skills.
- Proficiency in Microsoft Office and electronic documentation.
- Valid driver’s license and safe driving record.
