Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Weekend Rotation: Every 4th weekend (Friday & Monday off after)
Holidays: 2 per year
Status: Full-time (40 hours/week)
Job Summary
LPN's provides compassionate hospice and palliative care in home and care-center settings, supporting patients and families with clinical care, symptom management, and education.
Responsibilities
- Provide hospice nursing care (home visits, inpatient support, admissions, end-of-life care)
- Follow Plans of Care and physician orders
- Monitor patient condition and report changes to RN
- Educate patients and families on care and services
- Coordinate care needs, including DME
- Document care accurately in EMR
- Collaborate with interdisciplinary team
- Participate in weekend rotation

Qualifications
- Current Ohio LPN license
- Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health; hospice preferred)
- Valid driver’s license and reliable transportation
- Strong communication and time management skills
