Location: Western & Central Ohio
Schedule: 9:00 AM – 9:30 PM (set template days)
Weekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)
Holiday Rotation: 2 holidays per year
Hours: Full-Time (36 hours/week)
Pay: $35.00 – $40.00/hour (based on education, experience, and certifications)
Sign-On Bonus: $5,000 (for Full-Time position)
Position Summary
EverHeart Hospice is seeking a compassionate Admissions Registered Nurse (RN) to manage hospice referrals, intake, and patient admissions across Western and Central Ohio. This role conducts comprehensive admission assessments, determines hospice eligibility, and educates patients and families on hospice and palliative care services.
Admissions RN Responsibilities
- Conduct hospice admission assessments and verify eligibility
- Manage referrals, intake, and admissions
- Educate patients and families on hospice, palliative care, and insurance benefits
- Identify appropriate palliative interventions and ABN use
- Process physician and Medical Director orders
- Coordinate Durable Medical Equipment (DME)
- Provide skilled hospice nursing care, symptom management, and emotional support
- Support clinical operations during low admissions (home visits, end-of-life care, chart reviews)
- Ensure compliant, accurate documentation

Qualifications
- Current Ohio RN license
- Minimum 2 years RN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus)
- Valid driver’s license and reliable transportation
- Strong communication and time management skills
- Ability to work independently
