Schedule: Monday–Friday, 8:30 AM–5:00 PM
Job Description:
Evergreen Home Health is seeking a detail-oriented and organized Scheduler to coordinate patient and clinician schedules across our EMR and scheduling software. This position plays a key role in ensuring timely visit coordination, accurate scheduling, and smooth communication between the office, field staff, and clinical team.
The Scheduler will be responsible for coordinating schedules for starts of care, recertifications, 30-day evaluations, routine visits, and other required home health visits. This role requires strong attention to detail, the ability to manage schedule changes quickly, and consistent communication with clinicians and office staff to support efficient patient care.
Responsibilities:
- Coordinate and manage patient visit schedules in the EMR and scheduling software
- Schedule starts of care, recertifications, 30-day evaluations, routine visits, resumptions of care, discharges, and other required visits
- Ensure schedules are accurately maintained between the EMR and scheduling/routing software
- Communicate schedule updates, changes, and conflicts to clinicians and appropriate office staff
- Monitor upcoming visit requirements to ensure visits are scheduled timely and within required timeframes
- Assist with coordinating clinician assignments based on discipline, availability, location, and patient needs
- Make schedule adjustments due to cancellations, staffing changes, new referrals, or patient needs
- Track and follow up on unscheduled or incomplete visits
- Support coordination between nursing, therapy, and office teams to ensure patient care needs are met
- Help ensure schedules remain compliant with care plans, visit frequencies, and operational expectations
- Provide general administrative support to the operations and clinical teams as needed
Qualifications:
- Previous healthcare scheduling, home health, or administrative experience preferred
- Strong computer skills and ability to learn multiple software systems
- Strong attention to detail and organizational skills
- Ability to manage multiple priorities in a fast-paced environment
- Ability to problem-solve and adjust schedules quickly as needs change
- Excellent communication and teamwork skills
- Proficiency with Microsoft Office preferred
- Home health or clinical scheduling experience preferred but not required
