About the role

Evergreen Home Health is hiring a PRN Physical Therapist to provide in-home evaluations and follow-up visits. You choose any two days per week. We guarantee $500 per day, typically 4-5 patient visits/day. Ideal for PTs seeking flexible supplemental income with a supportive, responsive team.

What you’ll do

• Perform home-based PT evaluations; develop individualized plans of care.
• Deliver skilled therapy to improve mobility, safety, and independence.
• Educate patients/caregivers on exercises, equipment, and home modifications.
• Collaborate with the interdisciplinary team to coordinate care.
• Complete timely, compliant documentation in our user-friendly EMR.

Qualifications

• Active Michigan Physical Therapist (PT) license (Required)
• Home health experience preferred; will train the right candidate
• Strong communication, organization, and clinical judgment
• Reliable transportation and ability to travel within service area
• Comfortable documenting in an EMR

Schedule & pay

• PRN / Per-Diem, 2 days per week (days of your choice)
• $500 per day guaranteed; ~4-5 patient visits/day (typical)
• Mileage/other details discussed during interview

• 60 mile radius from Gaylord
