Are you an experienced MDS Nurse looking to make a meaningful impact? Our compassionate and dedicated team is seeking a motivated and detail-oriented MDS Nurse to lead the assessment and care planning process for our residents.
MDS Nurse What You’ll Do:
Coordinate and complete the MDS assessments and care plans in compliance with state and federal guidelines
Collaborate with interdisciplinary team members to ensure accurate documentation and individualized care
Monitor quality indicators and support initiatives for improving resident outcomes
Ensure timely submission of MDS assessments to maximize appropriate reimbursement
MDS Nurse What We’re Looking For:
Active RN license in the state of Ohio
MDS experience in a long-term care or skilled nursing setting (Required)
Knowledge of RAI process and PDPM guidelines
Strong organizational and communication skills
Ability to work independently and as part of a team
