We are seeking a dedicated and compassionate Director of Social Services to join our leadership team. This is a meaningful opportunity to make a difference in the lives of our residents and their families.

Director of Social Services Key Responsibilities:

• Oversee the Social Services department and ensure compliance with federal, state, and local regulations

• Conduct psychosocial assessments and develop individualized care plans

• Support residents and families through admissions, discharges, and transitions of care

• Coordinate services with interdisciplinary team members and outside agencies

• Provide leadership, supervision, and training for social work staff

• Advocate for residents’ rights and well-being

Director of Social Services Qualifications:

• Bachelor’s degree in Social Work or related field (Master’s degree preferred)

• Minimum of 2 years of experience in long-term care or healthcare setting

• Prior supervisory experience strongly preferred

• Knowledge of MDS, care planning, and discharge planning

• Strong communication, organizational, and leadership skills

• Licensed Social Worker in the state of Pennsylvania (preferred, but not always required depending on facility policy)
