Position Summary:
The In-Home Nurse will provide skilled nursing care to children with complex medical needs, such as G-tube feeding, tracheostomy care, oxygen therapy, seizure management, and other specialized interventions. This role requires strong clinical judgment, empathy, and dedication to delivering individualized care plans under the guidance of a supervising physician.
Key Responsibilities:
- Deliver one-on-one nursing care to pediatric clients in their homes.
- Monitor and assess patient health status and document changes promptly.
- Administer medications, treatments, and procedures as prescribed.
- Support families by educating them on medical routines and condition management.
- Collaborate with physicians, case managers, and the Enhanced Family Care team to ensure continuity of care.
- Maintain accurate, timely, and confidential patient records.
- Uphold all standards of infection control, safety, and ethical nursing practice.
Qualifications:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) with an active state license.
- 1–5 years of experience in pediatric or home health nursing (new graduates welcome with appropriate training).
- Strong clinical assessment and documentation skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and manage time effectively.
Preferred Skills:
- Experience caring for medically fragile children.
- Knowledge of Medicaid documentation and compliance requirements.
- Excellent communication and interpersonal skills.
