The Elizabeth Layton Center is seeking a qualified Individual Placement Support (IPS) Case Manager to serve our community. SUMMARY
The IPS Case Manager will provide case management services to consumers with mental illness who are seeking stable employment. This position will coordinate with clinicians to assist clients by providing information about employment, as well as connecting clients to employment agencies and/or opportunities with the goal of increasing the consumer’s independence, productivity, self-determination, integration, and inclusion in the community. The IPS Case Manager will primarily focus on providing services in the community, residence, vocational site, program site or other community settings.
MAJOR JOB RESPONSIBILITIES:
  • Facilitates care coordination between client and local businesses with the goal of obtaining employment for client
  • Builds and maintains positive relationship with local business and social services partners
  • Provides individual vocational services aimed at building skills related to employment, including preparation of applications, resume writing, interview skills, appropriate employment dress codes, interpersonal skills, and other skills including but not limited to: self-esteem, confidence building, motivation, assertiveness skills
  • Attends weekly individual and group supervisions
  • Attends mental health treatment team meetings weekly to support and empower client on symptom self-management so that the client can obtain and maintain employment
  • Develops an individual employment plan within 14 days of referral to the Individual Placement Support (IPS) program
  • Conducts on and off-site job coaching as indicated, and advocates for reasonable accommodations when needed
  • Collaborates with the Social Security Administration and a benefit planner to ensure consumer continues to receive benefits in compliance with work requirements
  • Stays informed about the current local job market, and makes 6 or more new contacts per week for job development
  • Meet with Vocational Rehabilitation Specialists monthly
  • Meets direct service expectations, deadlines for paperwork and reports, and provides quality documentation
  • Works in conjunction with other mental health center staff
  • Provides supportive intervention when needs arise outside of scheduled hours
  • Perform other duties as assigned by the supervisor or designee
  • Complies with ELC policies and procedures
EDUCATION and/or EXPERIENCE:
  • Bachelor’s degree or equivalent experience necessary to meet State of Kansas licensing regulations
  • Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards
  • Familiarity with mental health theory and practice
  • Current KS driving license with clean driving record
  • Incumbent must pass a security check as required by Medicaid Regulations
KNOWLEDGE, ABILITIES, AND SKILLS:
  • Relates a positive approach to complex mental health issues
  • Ability to communicate effectively orally and in writing
  • Ability to establish effective working relationships with staff, community agencies, and consumers
  • Ability to be sensitive to cultural and ethnic differences
  • Ability to safely perform major job responsibilities with or without reasonable accommodations
