Position Overview
The Medical Social Worker (BSW) provides support services to patients and families in the home setting under the supervision of a licensed Master’s-level Social Worker (MSW). Responsibilities include assessing social, emotional, and environmental needs related to the patient’s illness and care plan; assisting in discharge planning; connecting clients with community resources; and promoting optimal patient and family outcomes through support, education, and coordination of services.
Qualifications
Bachelor’s degree in Social Work (BSW) from an accredited college or university
Valid license or certification to practice social work in the state of Georgia, if required
Minimum of 1 year of experience in a healthcare setting preferred (home health, hospice, hospital, or related)
Excellent verbal, written, and interpersonal communication skills
Strong time management, organization, and documentation abilities
Essential Responsibilities
Conduct basic assessments of clients’ social and emotional needs and report findings to the supervising MSW or clinical manager*
Assist clients and families in understanding and coping with illness-related challenges*
Educate patients and families about available community resources and assist with access as needed*
Contribute to care coordination and participate in interdisciplinary care planning meetings*
Support the development and implementation of care plans under the guidance of the MSW*
Document services provided and maintain accurate, timely records according to agency policy*
Participate in discharge planning activities and ensure continuity of care support is in place*
Attend staff meetings, in-services, and training programs to maintain knowledge of best practices*
Adhere to confidentiality, HIPAA standards, and professional code of ethics at all times*

Physical Elements:
Ability to communicate clearly and compassionately with patients, families, and care teams
Manual dexterity to operate computers, telephones, and other office equipment
Visual ability to read charts, care plans, and documentation
Mobility to move throughout office settings and patient residences, lifting items up to 25 lbs as needed
Work Setting:
Combination of office and in-home visits
Exposure to diverse home conditions and environments, including potential hazards and emotional situations
