Responsibilities
- Provide skilled nursing care in patients’ homes
- Develop individualized care plans and manage symptoms
- Educate and support patients and families
- Collaborate with physicians, social workers, and other care team members
- Maintain accurate documentation
Qualifications
- Active RN license in Arizona (or compact-eligible)
- Minimum 1 year of clinical RN experience (hospice/home health preferred)
- Strong communication and critical-thinking skills
- Compassionate, adaptable, and patient-focused
- Valid driver’s license and reliable vehicle
