Community Care Coordinator
We are seeking a compassionate and detail-oriented Community Care Coordinator to oversee daily resident care, assist with Activities of Daily Living (ADLs), and manage caregiver scheduling. Key Responsibilities:
- Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility.
- Plan and post monthly schedules for CNA, Caregiver, Med-Tech, and Nursing staff.
- Adjust staffing for call-offs and emergencies to ensure coverage.
- Support recruitment efforts by scheduling interviews and assisting with onboarding.
- Greet visitors, answer calls, and coordinate communication between residents, families, and staff.
- Monitor resident health, respond to emergencies, and document changes.
- Maintain accurate health records, medication logs, and supply inventories.
- Participate in resident care conferences, activities, and welcome/orientation programs.
- Ensure cleanliness and organization of resident rooms, dining areas, and common spaces.
Qualifications:
- High school diploma or equivalent.
- CNA Certification (required).
- CPR/First Aid Certification.
- Strong communication, computer, and organizational skills.
- Ability to work in a fast-paced, team-oriented environment.
- 12 hours of annual continuing education (provided in-house).
