Responsibilities
- Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
- Assist residents with therapeutic exercises, strength, balance, gait training, mobility, transfers, and fall prevention.
- Monitor residents' responses to treatment and report progress or changes in condition to the supervising Physical Therapist.
- Carry out established treatment plans and communicate recommendations for modifications to the supervising Physical Therapist.
- Educate residents, families, and caregivers on exercises, safety, and assistive devices within the established treatment plan.
- Maintain accurate Net Health documentation and collaborate with the care team.
- Active, current Minnesota Physical Therapist Assistant license in good standing.
- Graduate of a CAPTE-accredited Physical Therapist Assistant program or an equivalent program accepted by the Minnesota Board of Physical Therapy.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, senior living, or rehabilitation experience preferred.
- Net Health EMR experience preferred.
