Pay rate will be between $18.50 to $20.50 per hour (and based on experience), and includes insurance and time off benefits
Full-Time position working 37.5 hours per week
Work schedule will be Monday through Friday, 7:30 am to 3:00 pm
Easterseals is seeking a dedicated and detail-oriented Case Manager to work collaboratively within the department to ensure consumer needs are met while supporting program objectives, quality assurance standards, and regulatory compliance. This role partners closely with the Center Director and ADT Supervisor/Assistant Director to support consumer training, documentation, intake, and administrative operations. The Case Manager plays a vital role in maintaining accurate records, coordinating with families and community partners, and delivering high-quality, person-centered services that align with our mission.

A snapshot of what you’ll be doing
- Maintain complete and compliant consumer files, documentation, service authorizations, and electronic records
- Coordinate intake, scheduling, and implementation of Individual Program Plans (IPPs) in collaboration with families, support coordinators, and community agencies
- Monitor attendance, progress reports, and skill development to support individual goals and program quality
- Ensure compliance with all regulatory, contract, funding, and accreditation requirements
- Collaborate with Center leadership to support consumer training, orientations, quality assurance, and regulatory reviews
- Assist with administrative operations including billing documentation, data entry, tours, and general office support
- Participate in community outreach and marketing efforts as needed
- Maintain required agency, regulatory, CPR, and First Aid trainings
Qualifications
- Associate’s degree (A.A.) or equivalent from a two-year college/technical school; or one year of related experience/training; or equivalent combination of education and experience
- CPR and First Aid Certification (or ability to obtain)
- Completion of required APD mandatory trainings
- 1–3 years of relevant experience in case management, adult day services, or elder services
- Knowledge of community-based resources for older adults and caregivers
- Strong communication, organizational, and problem-solving skills
- Proficiency in Microsoft Office, Adobe Acrobat, and internet-based tools
- An energetic, resourceful and professional individual who will promote a positive, respectful and team-based environment
- Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
