Description: ECC's Help Me Grow Program provides caregivers with access to free developmental screenings and ensures children and families are connected to needed community-based services and supports. The Care Coordinator assists caregivers with all follow up care, including initiating and coordinating referrals, supporting the caregivers with developmentally appropriate activities for the child, and connecting caregivers to programs in the community. The Care Coordinator will also assist the Community Outreach Coordinator by participating in community outreach events, promoting Help Me Grow information to child health care agencies, and representing Help Me Grow at interagency networking meetings and with other community organizations. A bilingual candidate is preferred.
Salary:
The starting salary is $44,500

Core Duties / Skills:
1.) Conduct age-appropriate developmental screenings.
2.) Develop kind, supportive, and empathic relationships with caregivers.
3.) Respond to calls and emails from caregivers and community agencies about child development and services.
4.) Effectively communicate screening results and recommendations to caregivers.
5.) Facilitate connection to resources and services in the community for children and families.
6.) Maintain neat, timely, and accurate case files, notes.
7.) Participate in local Help Me Grow meetings, interagency network meetings, and community outreach.
8.) Develop in-depth knowledge of Help Me Grow and local systems and services focused on early childhood development and special health care needs, as well as the eligibility, referral, and application processes for these services.
Work Environment(s):
This position has flexibility with working part of the time at our office location in Brandon, Florida and part of the time from home. Travel is required for scheduled monthly meetings and activities throughout Hillsborough County.
