EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Family Support Specialist for our new program, Nassau Family Resource Center.

Schedule: Flexible schedule of 35 hours per week with nights and weekends as required

What drives the core purpose of this job:
The Family Support Specialist serves as the primary point of contact for enrolled families, conducting intake and assessments while building ongoing, strengths-based relationships. The role focuses on connecting families to appropriate services through resource navigation, referrals, and follow-up, ensuring support aligns with each family’s needs and goals. The Specialist also supports program delivery and maintains accurate documentation and data entry.
What are the key responsibilities:

Conduct intake interviews and family assessments.

Utilize the Protective Factors Instrument (PFI) at baseline and three months post-enrollment.

Provide individualized resource navigation, referral coordination, and follow-up to ensure families are connected to appropriate services.

Facilitate support groups, assist with workshop preparation and logistics.

Enter all required data into the OCFS Prevention Database.

Establish and maintain ongoing, strengths-based relationships with families.

Ensure that services are responsive to each family’s identified needs and desired outcomes.
What key knowledge, skills, and abilities are required to excel in this role:

Bachelor’s degree in social work, psychology, human services, or related field preferred.

Minimum of 2 years relevant experience working with individuals and families.

Excellent communication skills, both oral and written.

Must be proficient in office and computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar program, and e-mail and/or database software skills necessary to perform job responsibilities.
