- Deliver compassionate primary care services to a diverse patient population.
- Diagnose and manage both acute and chronic conditions as a Family Nurse Practitioner.
- Conduct physical exams, wellness visits, and preventive screenings.
- Develop personalized care plans and educate patients on health management.
- Collaborate with physicians, nurses, and support staff to ensure coordinated care.
- Maintain accurate documentation in the electronic health record (EHR).
- Master’s Degree in Nursing (APRN).
- Active Family Nurse Practitioner (FNP) license in New York State.
- DEA certification with prescriptive authority.
- Minimum 1–2 years of Primary Care Nurse Practitioner experience preferred.
- Spanish-speaking skills are a plus.
- Strong clinical judgment, communication, and teamwork skills.
