What are the responsibilities and job description for the Care Coordinator/SUD Counselor position at Divine Healthcare Services, Inc.?
- Coordinate care with medical providers, mental health professionals, probation officers, social workers, and community agencies
- Assist clients with referrals for housing, employment, healthcare, transportation, education, and recovery support services
- Maintain accurate and timely documentation, case notes, progress reports, and discharge summaries
- Advocate for clients and assist with navigating healthcare and social service systems
- Develop individualized treatment plans and recovery goals in collaboration with clients
- Provide individual, group, and family counseling sessions
- Monitor client progress and update treatment plans as needed
- Work collaboratively with clients, treatment providers, community resources, and support systems to ensure continuity of care and successful treatment outcomes.
