What are the responsibilities and job description for the Substance Abuse Counselor position at Divine Healthcare Services, Inc.?
- Conduct client assessments and screenings to evaluate substance use history and treatment needs
- Develop individualized treatment and recovery plans
- Provide individual, group, and family counseling sessions
- Monitor client progress and maintain accurate clinical documentation
- Assist clients in developing relapse prevention and coping strategies
- Provide crisis intervention and emotional support when needed
- Educate clients and families about addiction, recovery, and community resources
- Collaborate with healthcare providers, probation officers, social workers, and other support agencies
- Maintain confidentiality and comply with HIPAA and ethical counseling standards
- Facilitate referrals to additional treatment, housing, employment, or support services
- Participate in staff meetings, trainings, and case consultations
Job Types: Full-time, Contract
