Job Summary
We are seeking a compassionate and dedicated Home and Community Support Specialist to provide exceptional care and assistance to individuals in their homes and community settings. This role involves supporting clients with daily activities, health monitoring, and emotional well-being, ensuring they receive personalized care that promotes independence and quality of life. The ideal candidate will have experience in home health, assisted living, or long-term care environments, with a strong focus on patient safety and dignity.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and record vital signs, patient observations, and overall health status
- Administer medications following prescribed care plans while adhering to HIPAA regulations
- Provide meal preparation and nutritional support tailored to individual needs
- Support clients with cognitive impairments including Alzheimer's disease, dementia, autism, or developmental disabilities through specialized care techniques
- Operate assistive devices such as Hoyer lifts and ensure safe transfer procedures
- Maintain a clean and safe environment by performing light housekeeping tasks as needed
- Document care activities accurately in compliance with healthcare standards and regulations
- Collaborate with healthcare professionals to develop and implement personalized care plans
- Respond promptly to emergencies with appropriate first aid or CPR when necessary
- Support emotional well-being through companionship and social engagement activities
Experience
- Prior experience in home health, assisted living, nursing home, hospice care, or social work preferred
- Knowledge of patient monitoring, medication administration, and vital signs measurement
- Experience working with individuals with disabilities, developmental disabilities, Alzheimer's/dementia care, or autism is highly desirable
- Familiarity with HIPAA regulations and confidentiality standards in healthcare settings
- Hands-on experience with patient observation, caregiving techniques, and safety protocols such as heavy lifting and resident transfers using Hoyer lifts or similar equipment
- Background in long-term care or senior care environments is advantageous
- Certification in CPR and first aid is preferred but not required; training can be provided during onboarding
