Resident Assessment Coordinator

We are seeking a highly skilled and experienced Resident Assessment Coordinator (RAC) to join our team at Dickson Health and Rehab in Dickson, TN. As an RAC, you will play a crucial role in ensuring the accuracy and timeliness of resident assessments, care planning, and reimbursement optimization, while supporting quality resident outcomes in a long-term care setting. This is a full-time position with a salary range of $30-$35 per hour.

Key Responsibilities:

• Complete and submit accurate and timely Minimum Data Set (MDS) assessments (admission, quarterly, annual, significant change, discharge)
• Coordinate interdisciplinary care plan meetings
• Ensure comprehensive, individualized care plans are developed and updated
• Maintain compliance with RAI Manual guidelines
• Ensure compliance with CMS, Medicare, Medicaid, and state regulations
• Maintain survey readiness related to MDS accuracy and documentation
• Optimize reimbursement through accurate coding and documentation
• Monitor PDPM components and case mix index (CMI)
• Collaborate with therapy, nursing, and billing teams
  
  
  

Qualifications:

• Current RN license in Tennessee
• MDS experience in long-term care preferred
• Strong knowledge of RAI Manual and PDPM
• Experience with electronic medical records (EMR) systems
• Strong organizational and time management skills
  
  
  

Preferred Qualifications:

• MDS Certification (RAC-CT or similar)
• Experience with Medicare/Medicaid reimbursement systems
• Prior experience in skilled nursing facility (SNF) setting
