The RN Cardiac Case Manager provides nursing services and care coordination to high risk and/or high cost patients with acute or chronic disease processes. Services are provided telephonically. If the service is being provided for specific disease states, the service will be provided telephonically and at specific DOHC/DOHC contracted locations. The RN Cardiac Case Manager provides care coordination, education, counseling, and on-demand information to patients who have been enrolled in a condition specific program. This role contacts individuals with acute/chronic disease and influence their healthcare outcomes. The RN Cardiac Case Manager is responsible for coordinating certain operations, including collaboration of services with the patient, physicians, nursing staff, case managers, social workers, home health, infusion services, and other services offered by the medical group to facilitate management of the patient.

Professional Duties
  • Perform one in-service annually
  • Average phone time of 1 to 3 hours on a daily basis.
  • Actively seeks supervisory assistance when issues arise that fall outside scope of practice or experience.
  • Collaborates with other departments as needed to meet patient needs.
  • Responsible for telephonically monitoring members by managing their healthcare conditions, prioritize their healthcare needs, and prevent complications.
  • Meet member’s needs and provide informational services to those assigned members with focus on points of entry into health care services. (i.e. Primary Care Provider, Immediate Care, Member Services)
  • Will document and communicate effectively the Problem, Intervention, and Plan within the current Electronic Medical Record. This documentation must meet the needs of the medical group by supporting the requirements mandated by the health plans in order to meet routine audits. Documentation to include routine updates to the plan of care.
  • Completes Clinical Social Assessments regularly and routinely throughout the year. This will be done at a consistent rate each month so as to reach a 100% completion rate before the end of the year. POLSTS and DPOAs will be discussed with members regularly. Will also routinely address the importance of preventative care and the need for appropriate follow up with those Health Care Professionals rendering health care with focus on needed quality measures.

Qualifications
  • Graduate of an accredited nursing program.
  • Current California Registered Nurse (RN) license.
  • 3 years of recent nursing experience in cardiology, telemetry, critical care, home health, or case/disease management required.
  • Strong medical knowledge and competence in chronic disease pathophysiology, polypharmacy, clinical assessment, and patient-family education.
  • Ability to work independently with a minimum of direction on a high volume caseload environment. Ability to anticipate/organize workflow, prioritize and follow through on responsibilities. Comfortable having work reviewed and measured.
  • Positive interpersonal, influencing and decision making skills. Ability to work constructively with a broad spectrum of health care professionals. Ability to work respectfully and creatively with clients of diverse functional abilities, social, economic, and cultural backgrounds to support both client autonomy and client safety.
  • Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others. Ability to negotiate and resolve conflicts.
  • Ability to utilize Microsoft Office Suite applications/software (Word, Excel, Power Point, Excel) and other computer software/applications. Ability to utilize electronic medical/health record system.
  • Detail oriented and organization skills. Ability to manage time effectively and prioritize tasks to meet established deadlines.
  • BLS required within 60 days of hire.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
Physical Demands
  • Sitting: Approximately 75% of day.
  • Standing: Approximately 25% of day.
  • Walking: Approximately 25% of day.
  • Lifting: 0 - 10 lbs. approximately 25% of day.
  • Bending: Approximately 15% of day. Kneeling approximately < 20% of day.
  • Hearing/Visual Acuity: Adequate for use with computers, telephone/mobile device and other office equipment approximate 50% of day.
  • Computer: Highly technical work environment. Must be able to work 7 hours per day using computers, telephone/mobile device and other office equipment.
  • Reaching: Above head 75 degrees approximately 5%.
  • Hand grip dexterity: Approximately 70% of day.
