What are the responsibilities and job description for the Quality Performance Coordinator position at Desert Oasis Healthcare?
The Quality Measure (QM) Performance Coordinator will support the overall efforts of the Clinical Performance and Outcomes team to ensure Desert Oasis Healthcare Quality Measure performance goals are met on an annual basis (senior, commercial and ACO quality measures). These activities will include contacting patients with quality measure gaps and utilizing customer service, care coordination and motivational interviewing techniques to promote compliance with quality measure requirements. Research of patient medical records and documentation of activities in the EMR will be required. Will provide support to physician offices regarding quality measure requirements and will be required to assist in ongoing provider office education.

- Will provide outreach efforts to DOHC members in the interest of boosting quality measure performance. Will work with the DOHC Clinical Performance and Outcomes team to identify opportunities and determine priorities for specific efforts and initiatives. These will include telephonic and-or written patient specific outreach efforts (via mail or patient portal).
- Will work interdepartmentally to ensure patient completion of quality measures is incorporated into overall coordination of care activities. This will include Labcorp, QHMO and DAI scheduling of appointments.
- Supports the Provider Liaison for QM Performance in the provision of quality measure education and training at primary care physician offices. Assists in generating regular reports and communication to primary care offices regarding quality measure performance.
- Assists the Provider Liaison for QM Performance with the investigation and resolution of issues and concerns relating to quality measure compliance that may be raised by the primary care physician office. May work directly with corporate office or the health plan to resolve the issue.
- Will develop extensive knowledge of QHMO or any other internet based tools that may be used to assist in boosting quality measure performance.
- Will document all activities in the EMR.

- High School diploma, Medical Assistant background preferred.
- At least one year experience in a medical group or medical office setting.
- Experience with data entry.
- Computer knowledge (Word, Excel).
- Must be neat, accurate and detail oriented.
- Good interpersonal communication skills.
- Must possess excellent writing and grammar skills (bilingual preferred)
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
