Responsibilities Include:
Performing skilled nursing visits in patients' homes
Assessing patient needs and developing individualized care plans
Monitoring patient progress and communicating updates to physicians and the interdisciplinary team
Administering medications, treatments, and skilled nursing interventions
Educating patients and caregivers on disease management, safety, and home care techniques
Completing accurate, timely, and compliant clinical documentation
Qualifications:
Current Arizona RN license in good standing
Home Health nursing experience required
Valid driver's license, reliable transportation, and current auto insurance
Strong assessment, clinical decision-making, and communication skills
Ability to work independently while collaborating effectively with the care team
