What You’ll Be Doing
- Assess patients’ psychosocial needs and develop care plans
- Provide counseling and emotional support to patients and families
- Assist with discharge planning and community resource coordination
- Advocate for patient needs with the interdisciplinary care team
- Educate patients and caregivers on available services and support systems
- Maintain accurate and timely documentation
- Ensure compliance with HIPAA and regulatory standards
What You Bring
- Master’s Degree in Social Work (MSW) required
- Current state licensure (as required)
- Minimum 1–2 years of healthcare experience preferred
- Home Health or Hospice experience preferred
- Strong knowledge of community resources and care coordination
- Excellent communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
