Key Responsibilities:
Patient Assessment – Conduct comprehensive evaluations to determine home care needs.
Care Planning – Develop and implement personalized care plans tailored to each patient’s unique situation.
Team Coordination – Lead and collaborate with care teams to align with patient goals.
Patient Education – Provide guidance and support to patients and families for informed healthcare decisions.
Clinical Documentation – Maintain accurate records and communicate updates with primary physicians.
What We’re Looking For:
Licensure – Active and valid Arizona RN license.
Experience – Minimum of one year in home health care; OASIS documentation proficiency preferred.
Transportation – Valid Arizona driver’s license and reliable transportation.
Certifications – Current CPR certification and Arizona Fingerprint Card.
Language Skills – Bilingual or conversational Spanish highly preferred.
Education Requirements:
Degree – Associate's Degree in Nursing or higher.
