Your Role:
Provide personalized, compassionate care in alignment with care plans and physician orders
Work closely with our Nurse Case Manager and interdisciplinary care team
Communicate clearly with patients, families, physicians, and staff
Monitor and report on physical, emotional, and social health factors
What You Bring:
Active LPN/LVN license in Arizona
Excellent communication and organizational skills
Insured, reliable vehicle and valid driver's license
Current BLS certification
Home Health experience and bilingual (English/Spanish) preferred
Fingerprint Clearance Card (or willingness to obtain)
