Your Role
- Conduct in-home patient assessments and develop personalized care plans.
- Administer medications, wound care, and treatments as prescribed.
- Educate patients and families on managing health conditions and promoting independence.
- Monitor patient progress and update care plans as needed.
- Supervise LPNs and Home Health Aides to ensure the highest quality of care.
- Collaborate with an interdisciplinary team to coordinate holistic patient care.
- Active Registered Nurse (RN) license in the State of Arizona.
- Minimum of one year of nursing experience (home health, hospice, or critical care preferred).
- CPR certification and valid driver’s license with reliable transportation.
- Excellent communication, organizational, and interpersonal skills.
