Your Role
As a Home Health PT, you’ll provide in-home physical therapy to patients recovering from injuries, surgeries, or medical conditions, helping them achieve their mobility and wellness goals.
Key Responsibilities
- Evaluate patient needs and develop custom treatment plans to improve mobility, relieve pain, and enhance function
- Provide therapeutic exercises, manual therapy, and other interventions for recovery
- Educate patients and families on the importance of therapy and support them throughout the recovery journey
- Collaborate with healthcare teams including physicians, nurses, and OTs to ensure seamless care
- Document patient progress accurately to support coordinated care and positive outcomes
- Stay current on professional development and industry best practices
- Licensed Physical Therapist (PT) in Arizona
- Strong communication skills and the ability to build trust and rapport with patients and colleagues
- Ability to work independently and as part of a team in a dynamic setting
- Analytical and problem-solving skills to adjust treatments for optimal outcomes
