Job Summary
This position is located within the District of Columbia Government (District), Department of Aging and Community Living (DACL), within the Community Transition Program. The mission of DACL is to advocate, plan, implement, and monitor programs in health, education, and social services for the elderly and those with disabilities.
The incumbent is responsible for overseeing and directing the Community Transition Outreach program and activities that provide person-centered transition services to District senior residents and residents with disabilities.
Duties And Responsibilities
Oversees the Community Transition Outreach Program that provides person-centered planning, which includes partnerships with advocates, government officials, non-profit-and for-profit interest groups and organizations addressing residents seeking to transition into the community. Creates, implements, and maintains all outreach messaging models, materials, data collection, provides comprehensive assessments to nursing home residents, family members, legal representatives, long-term care professionals, as well as the public regarding nursing home discharge procedures and person centered community transitions. Conducts monthly staff community outreach meeting and ensures team members build collaborative relationships with key representatives of DC Medicaid supported licensed nursing home facilities in the District.
Ensures program staff develop working relationships with nursing home residents, caregivers, family members, legal guardians, long-term care advocates, community case management and home health agencies to ensure understanding of community transitions services. Directs program outreach communication and messaging for public understanding of DACL community transition services, other home and community-based services, and both District and Federal programmatic expectations. Directs the allocation of resources to achieve timely outcomes and measurable goals within budget; adjusts plans and programs to meet emerging or new programs while continuing to address major community transition program priorities. Performs the full range of management and supervision of staff, to include training and daily support and direction, evaluation of employee performance, including making recommendations for disciplinary actions; approves or denies subordinate leave, etc.
Qualifications And Education
Specialized Experience: Specialized experience must have equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position to be filled. To be creditable, applicants must have one (1) year of specialized experience equivalent to at least the next lower grade level in the line of progression.
An example of Specialized Experience is: working with a senior or person with a disability in their home, community or hospital, long-term care or nursing home setting to obtain services; working with clients and their care givers evaluating and coordinating social service options; working with clients in long-term care facilities to address individual transitioning needs, professional advocate and legal requests, and to address basic community responses during the nursing home discharge planning process; or similar work experience.

The work environment is both office and community settings. It requires frequent local travel to various nursing facilities, long-term care and assisted living facilities, off-site events, training, and other community-based settings.

MSS At-Will Statement: Positions in the Management Supervisory Service (MSS) serve at the pleasure of the appointing authority and may be terminated at any time with or without cause.

As such, individuals in this position may be required in emergency situations to perform crucial duties that are vital to the continuity of mission crucial functions and operations. These duties may be performed when the government is closed or when most employees are dismissed early. Employees designated as “emergency” may be required to perform other duties outside their official position during a period of a declared emergency.
