Responsibilities
Responsible for providing assessment, linkage, and advocacy services to individuals from birth to age three with a developmental delay, atypical development, or diagnosed disabling condition recognized by the Part C Early Intervention system. The Early Childhood Case Manager - Contract will act as primary facilitator in the coordination and integration of services for assigned caseload; develop and implement Individualized Family Service Plans (IFSPs); complete required documentation; and comply with all regulations applicable to program services. Applies generally prescribed standard practices over a wide range of procedures with some latitude for decision making.
Requirements
Minimum Requirements
M1:Bachelors Degree in Early Childhood Education, Special Education, Human Services related field, or any discipline or profession listed under Part C Personnel Standards
OR
Registered Nurse with a valid license to practice in Virginia
M2: Within two months of employment, must complete Early Intervention Training Modules, apply for, and obtain Early Intervention Case Manager certification
M3: Valid drivers license and safe driving record
Preferred Requirements
P1:Prefer that applicant meet Early Intervention Professional Practitioner Level for a discipline or profession as defined in Chapter 12 of the Infant and Toddler Connection of Virginias Practice Manual (Table C Practitioner Qualifications and Responsibilities).
P2: Prefer at least one year experience working with children under the age of three with developmental delays or disabilities.
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
