The Dane County Human Services Department, Disability and Aging Services division, has a full-time (1.0 FTE) vacancy for an Information and Assistance Lead Specialist in Madison, WI.

DEFINITION

Under the supervision of the Information and Assistance Supervisor, provides information and assistance, connecting older adults and adults with disabilities and caregivers to programs and services, including public and privately funded options. Provides assessment and long-term care options counseling services to older adults and adults with disabilities in Dane County. Offers Options Counseling built on the strengths of individuals, families, and the community. Works collaboratively with internal and external services in order to advocate for consumer needs to assure services are delivered.  Will also perform lead work duties.


EXAMPLES OF DUTIES

Provides consumer-driven prompt, accurate, and friendly customer service responses to requests for information about an array of topics and for access to long term care resources. Searches through the Information and Assistance resource database, as well as other written and computer-based information resources to identify, evaluate, and suggest potential programs and services. Collects consumer information and assesses client/family needs in order to provide information and assistance over the phone, in the office, or at home that best meets the needs/demands of the consumer’s schedule. Provides follow up as needed to determine outcomes and provide additional assistance in locating resources and arranging services. Develops and implements consumer-driven action plans as part of short-term service coordination, to organize and direct in-home supportive services for individuals with a limited support network, according to ADRC policy. Assists in the identification of individualized, cost-effective, outcome-oriented service options (Options Counseling) that reflect goals that are personally meaningful and attainable by consumers and address immediate as well as longer term needs. Administers the Long-Term Care Functional Screen to determine functional eligibility. Provides information about publicly funded long-term care and assists consumers throughout the eligibility determination and placement on waitlist for services, and future enrollment process Collects preliminary financial data (including medical and remedial expenses) and refers to Economic Support Unit for financial eligibility determination. Assists individuals in completing applications for public benefit programs. Participates in various transition activities designed to help youth who have a disability transition from school to the adult service system. Participates in prevention activities to include: evidence based classes, support groups, and/or presentations. Problem solves with internal and external agencies on any concerns with clients as they relate to the independence and needs of the consumers served. Assists Dane County Emergency Management during a community emergency/disaster according to the ADRC Emergency Preparedness Plan. Participates in Quality Assurance/Quality Improvement projects and activities. Completes and maintains appropriate and timely documentation on consumers in the consumer records and databases. Completes and maintains required agency documentation for 100% Time Reporting purposes. Participates on special task forces, internal and community committees, in order to assist coordination of programs and services. Provides outreach activities in the form of booths and/or public speaking engagements for educational purposes on aging and disability services in the community. Represents ADRC of Dane County to the community at large through professional interaction, public speaking, media presentations, and participation in community advisory groups as requested. Adheres to Inform USA national standards. Provides mentoring, leadership and direction to the Information and Assistance general staff by instructing and training on staff procedures, and providing leadership for completing a specific task and assisting others in meeting deadlines. Provides staff support, training, mentoring, and back-up staff coverage. Assists Information and Assistance general staff with difficult or challenging cases. Other duties as assigned.


 

Education, Training and Experience:

Any combination of education, training and/or experience equivalent to a Bachelor’s Degree from an accredited college or university in a human service-related field or registered nursing field, and two (2) years of experience in an aging and/or disability related professional position.  


Special Requirements:

Valid Wisconsin driver’s license (or eligibility for), access to personal transportation Options Counseling certified within 2 months of hire, and Functional Screen Certification within 3 months of hire.  Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.  Maintain driver’s license, access to reliable personal transportation and functional screen certification and options counseling certification while employed in this position.

Wisconsin's Fair Employment Law, s. 111.31 – 111.395, Wis. Stats., prohibits discrimination because of an arrest or conviction record. For Bilingual Positions:

  1. Provide services in both English and the required foreign language (e.g., Spanish, Arabic, Hmong, etc.). 
  2. Applicants being considered for bilingual positions will be tested during the recruitment process prior to employment. This will primarily consist of passing an oral language proficiency assessment from English to foreign language and foreign language to English, but may require additional language testing, such as written translation, if it is an essential function of the job. 

*If applicable, individuals may confirm with the Employee Relations Division whether results of bilingual language certifications/ examinations they currently hold qualify them for the position’s bilingual designation.


Physical and Environmental Work Requirements:

Work involves walking, sitting, standing, bending, stooping, twisting, turning, lifting, carrying, pushing, pulling, grasping, and reaching within normal ranges. Ability to lift and move a variety of work-related materials weighing up to 25 pounds. Prolonged computer and phone use. Manual dexterity using fine and gross motor skills is necessary. Ability to speak and be heard by others. Ability to see and hear within normal ranges with or without corrective devices.

Knowledge of and demonstrated compliance with professional ethics and boundaries standards and codes of conduct. Knowledge of standards and principles of Information and Assistance. Knowledge of community resources and referral procedures. Knowledge of assessment and care management techniques. Knowledge of planning, organizing, and problem solving strategies. Knowledge of public service agencies, laws, and regulations governing or affecting aging and community services. Knowledge of HIIPPA, privacy, and confidentiality standards of practice. Ability to relate to older people and people with disabilities and assess their needs. Ability to deal tactfully with difficult situations. Ability to communicate effectively, both orally and in writing including the ability to communicate effectively to a variety of types of persons, both individually and in groups. Ability to proficiently use a computer and related office equipment. Demonstrating familiarity with required software and data base programs. Ability to establish priorities for service intervention Ability to be decisive and to make sound decisions under the pressure of crisis or emergency situations. Ability to participate in a work setting as a team player. Ability to successfully lead others.
