Responsibilities
- Develop and implement individualised care plans tailored to each patient's needs, including those with dementia, Alzheimer's, or requiring hospice or ICU care.
- Coordinate multidisciplinary teams to deliver seamless patient care across assisted living facilities, nursing homes, and home care settings.
- Supervise and manage healthcare staff, providing guidance and support to ensure compliance with clinical standards and policies.
- Administer medications accurately and safely in accordance with prescribed treatments.
- Conduct assessments of patients’ conditions, utilising knowledge of anatomy, laboratory results, and medical imaging to inform care strategies.
- Provide expert guidance on specialised areas such as dementia care, Toddler care (for patients with disabilities), and working with people with disabilities.
- Monitor patient progress and adjust care plans as necessary to optimise health outcomes.
- Ensure compliance with all healthcare regulations and maintain meticulous documentation of patient records.
- Educate patients and their families about health management strategies, including medication administration and managing chronic conditions like Alzheimer’s disease.
- Duration: 13 Weeks
- Schedule: 4x10s (Day Shift | 08:00–18:30)
- Hours: 40 hours/week
Experience
- Proven experience in assisted living environments or nursing homes is essential.
- Demonstrable background in senior care, hospice services, ICU environments, or dementia/Alzheimer’s care is highly advantageous.
- Experience working with children or toddlers with disabilities will be considered a plus.
- Minimum 2 years of Case Management experience BLS & BSN required Strong Epic experience required (Charting System) Patient Population: Adolescents, Adults, Geriatrics
