- Licensed Social Worker in Alabama (Preferred)
- Degree or education in Social Sciences
- Long-term care experience preferred
- Strong communication, documentation, organization, and follow-up skills
- Compassionate, dependable, flexible, and resident-focused
- Serve as the advocate and liaison for residents’ social and emotional well-being.
- Plan, organize, implement, and evaluate the Social Services Department.
- Assist residents and families with admissions, discharge planning, care transitions, and available community resources.
- Ensure residents’ medically related emotional and social service needs are identified and addressed on an individual basis.
- Maintain compliance with all federal, state, and local regulations governing long-term care facilities.
- Collaborate with the interdisciplinary care team to promote the highest quality of resident care and quality of life.
- Complete timely and accurate documentation while maintaining resident confidentiality.
