Position Summary
This role is a case management role focused on supporting disabled individuals who are a part of a supported housing program to increase housing stability within the community. Works as a part of an interdisciplinary team with CrescentCare and/or community housing providers to provide education, service planning, and housing coordination services to clients with a focus on increasing housing stability through utilization of mutually agreed upon housing plans. Maintains a close working relationship with direct supervisor, participates in active supervision and remains receptive to feedback on professional development to support high quality service provision.
Key Duties and Responsibilities
- Provides housing case management support to clients on the assigned caseload so that they might maintain their housing.
- Engage clients approximately 5 hours a day.
- Submits well-written notes based on services and other required paperwork within required deadlines.
- Assess clients’ needs and understand available resources.
- Develop a crisis plan with the client.
- Link clients to the available supportive services and resources.
- Coordinate services with other members of your care team, community partners and funders.
Qualifications
- Currently Licensed social worker (RSW, CSW, LMSW, LCSW) from a program accredited by the Council on Social Work Education
- The minimum education level required for the position is a BA/BS.
- The minimum experiential level required for the position is three years providing social services.
- Must have reliable transportation and a valid driver’s license, no license denials, revocations, or more than one suspension in the past three years; valid car insurance; no more than one at-fault accident in the past three years and no more than two moving violations in the past five years.
