POSITION SUMMARY
Provide culturally appropriate prevention department services and referrals to clients coming through the CrescentCare’s prevention program and conduct prevention outreach to the population. Assist in developing campaigns, health messaging, and recruitment of priority populations to prevention services. Conduct HIV/STI/HCV and PrEP counseling and testing. Assist lead transportation coordinator in organizing and supporting departmental outreach transportation needs. Develop and maintain relationships with community partners for outreach. Assist in and participate in all MHSD data collection and reporting. Support prevention department programs.
KEY DUTIES AND RESPONSIBILITIES

Assist prevention manager and outreach coordinators with outreach implementation

Provide prevention service navigation and support to outreach participants

Conduct HIV/HCV/STI counseling and testing

Assist in data collection and data entry, including electronic medical record extraction and follow up surveys

Conduct community outreach, focus groups, and educational events, in community venues and social events in urban locations

Develop and maintain relationships with community outreach partners.

Complete all reporting requirements in timely and professional manner

Ensure compliance with funder requirements for prevention programs

Other duties as assigned
POSITION REQUIREMENTS

High School degree/GED/HiSet AND 2 years’ experience in public health or relevant field (social services, non-profit, research, client navigation, community health work and/or community engagement) and competency with excel/data entry/computer literacy

OR Bachelor’s degree and competency with excel/data entry/computer literacy
