Job Responsibilities:
- Provide case management to 15-20 adults who are working towards recovery.
- Work within a team that consists of a Recovery Support Team Leader, Recovery Coaches, Certified Peer Specialist and various Clinicians.
- Provide direct care including assistance in obtaining and maintaining housing to consumers to support and promote recovery.
- Provide assessment and referral tools for people in order to support their recovery.
- Facilitate the acquisition of knowledge, skills and abilities for people in recovery.
- Facilitate the completion of recovery plans that are strengths based, recovery focused and individualized to the hopes, dreams, and aspirations of people in their recovery.
- Facilitate the creation of social networks outside the program for people in recovery.
- Maintain accurate record keeping and documentation of all contacts.
- Complete all billing and required paperwork in a timely manner.
- Provide on-call services when applicable.
- Pursue ongoing professional and personal development meant to ensure on-going growth.
- Carry out directives by Recovery Support Director and Recovery Support Team Leader.
Job Requirements:
- A Bachelor's Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, criminal justice, theology, nursing, counseling or education.
- Be a registered nurse.
- A High School Diploma and 12 semester credited hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in public or private human services with 1 year in direct client contact.
- A High School Diploma and 5 years of mental health direct care experience in public or private human services with employment as an intensive case management staff person prior to April 1, 1989.
- Be supervised by a mental health professional. A record of supervision shall be on fill.
- Must have a valid Driver's License.
