The co-location of primary care services along with behavioral health treatment offers significant advantages to the community. Position Summary: This position offers support and assistance to inspire hope in individuals that recovery is not only possible, but probable. It's helping others in their recovery and community integration process designed on the principles of consumer choice and the active involvement of persons in their recovery process.

Duties and Responsibilities:

  • Peer Recovery Specialist services shall be available at all stages of the recovery process including the pre-contemplative stage and those not yet engaged in any type of treatment and/or services as well as those who have completed a treatment protocol and are in need of additional and ongoing supports in the community.
  • Assist the client in identifying strengths and abilities to teach a person in recovery how to utilize personal assets to advance recovery.
  • Assist the person in identifying non-treatment related services that can be identified and addressed. The tool(s) to be used to assist in this activity is the Non Treatment Needs Checklist and the aftercare plan for individuals discharged from treatment.
  • Identify community/natural resources and assist in connecting with them and other peer supports such as mutual aid groups.
  • Offer guidance on the recovery experience to include stage appropriate recovery education.
  • Provide ongoing assistance in obtaining and/or maintaining services from multiple systems.
  • Support persons in recovery who are trying to connect/reconnect with family, friends, significant others and to improve or eliminate unhealthy relationships.
  • Referrals to Peer Recovery Specialist can be made by self-referral, Case Managers, Contracted Providers, Human Service Agencies, Probation/Parole. entry into Recovery Support services is voluntary.
  • A file will be opened on each recoveree agreeing to receive services.
  • Attempt contact with the recoveree within five (5) business days of the referral. Failure to meet this time frame will be documented in the recoveree's chart. All contact attempts (minimum of 3) will be documented on the referral. If the recoveree is unreachable, the referral source will be notified.
  • Review initial referral form/complete one and explain services, build rapport and begin to engage the recoveree. Recoveree will be offered a copy of the Peer Recovery Explanation of Services and will be asked to sign and date the Consent to Service form and any other applicable release(s).
  • All refusals will be documented on the referral form, retained by the Case Management office and no file will be opened. The referral source will be notified when an appropriate release of information is on file.
  • PRS will follow the laws, rules, regulations, and procedures for confidentiality.
  • Services will be provided through a variety of resources in the areas of Coordination of Care, Casework Activities, Outreach and Aftercare, The recoveree's needs and willingness to participate will determine services provided. Those services may include but are not limited to the following:
  • Assistance in accessing various community resources
  • Guidance on the recovery process and recovery education
  • Help with developing a personal recovery plan
  • Assistance in building life skills
  • Telephone recovery checkups
  • Introduction and support through engagement of recoverees in the Recovery Community.
  • Support before, during and after treatment.
  • Advocacy; outreach for recoverees in early recovery
  • Recovery support groups
  • Referrals to case management services when additional treatment services are needed.

Qualifications:

  • 2 years clean and sober
  • HS/GED graduate
  • Will have a Certified Peer Specialist credential within 12 months of hire.
