Position Overview
Our client is seeking a Pharmacist to provide clinical pharmacy services for patients receiving home infusion therapy.
You’ll oversee medication therapy, IV compounding, patient monitoring, education, and coordination of pharmacy services while collaborating with physicians, nurses, and other healthcare professionals.
What You’ll Do
Evaluate patients and prescribed therapies for home infusion care. Develop and maintain individualized pharmacy care plans. Review labs, perform pharmacokinetic dosing, and make clinical recommendations. Dispense medications, supplies, and infusion equipment. Educate patients and caregivers on medications and infusion therapies. Monitor patient progress, compliance, labs, and treatment outcomes. Ensure compounding and clean-room activities meet USP standards. Coordinate refills, supplies, labs, compounding, and deliveries. Supervise and support pharmacy technicians and other pharmacy personnel. Participate in on-call responsibilities as required.
