The Pharmacist in Charge will collaborate closely with physicians, nurses, and other healthcare professionals and report to the Executive Team.
What You'll Do
- Manage pharmacy operations and ensure pharmaceutical care is delivered in accordance with company policies, accreditation standards, and applicable regulations.
- Lead pharmacy staff, including hiring, training, performance management, and monitoring key performance indicators.
- Evaluate and monitor drug therapy based on the total needs of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
- Ensure compliance with applicable state and federal laws and regulations governing pharmacy practice.
- Maintain accreditation standards and participate in quality assurance activities.
- Coordinate new referrals and patient admissions with nursing, customer service, and distribution teams.
- Review, receive, and verify new patient medication orders and maintain appropriate documentation.
- Partner with physicians and other healthcare professionals to develop patient care plans, establish therapeutic objectives, define monitoring parameters, and prepare progress reports.
- Conduct and document initial and ongoing patient assessments to identify actual or potential drug-related problems.
- Monitor drug therapy outcomes through laboratory results, clinical data, and other relevant patient information.
- Provide medication consultation to patients, families, physicians, and other healthcare professionals.
- Maintain thorough documentation of clinical activities and communications in patient records.
- Process and verify medications, supplies, and equipment prior to dispensing.
- Maintain current clinical knowledge and pharmacotherapy skills through ongoing professional development.
- Bachelor’s degree in Pharmacy or PharmD required.
- Active Registered Pharmacist license in Iowa.
- Proven experience evaluating and monitoring drug therapies for home care patients.
- Strong organizational and time-management skills.
- Excellent verbal and written communication skills.
- Ability to work collaboratively with physicians, nurses, patients, families, and other healthcare professionals.
- Participation or membership in professional pharmacy societies and organizations.
- Base salary of $160,000–$175,000.
- Annual bonus opportunity of $5,000–$10,000.
