Licensed Practical Nurse - Supervisor

The LPN Supervisor is responsible for supporting the daily clinical operations of our Personal Care department while providing leadership and supervision to nursing staff. This position works closely with the Nursing Manager to ensure residents receive high-quality, compassionate care in accordance with physician orders, Department of Human Services (DHS) regulations, and facility policies.

Responsibilities

• Supervise and support Nursing Assistants, Medication Technicians, and LPN staff within the Personal Care department.
• Ensure physician-ordered treatments, medications, diets, and other healthcare services are properly administered and documented.
• Collaborate with the Nursing Manager to oversee daily nursing operations and staff performance.
• Participate in the nursing department's on-call rotation to provide after-hours support when needed.
• Assist with scheduling to ensure appropriate staffing coverage 24 hours a day, 7 days a week.
• Provide coaching, mentoring, performance evaluations, and corrective guidance to assigned staff.
• Assess resident health status, identify changes in condition, and provide appropriate nursing interventions.
• Accurately document resident care, assessments, and clinical updates in PointClickCare and other required records.
• Review and reconcile electronic Medication Administration Records (eMAR) and Treatment Administration Records (TAR) to ensure medication accuracy and resident safety.
• Coordinate appointments, laboratory services, pharmacy services, and other outside healthcare providers.
• Arrange transportation for resident medical appointments as needed.
• Maintain resident medical records, administrative documentation, and required clinical files.
• Process clinical communications, including faxes, laboratory reports, and pharmacy correspondence.
• Coordinate with Central Supply to obtain necessary medical supplies and equipment.
• Assist residents with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as needed.
• Promote resident engagement through recreational, cognitive, physical, and social activities.
• Assist during meal service when needed to support resident care.
• Assist with monthly fire drills and other safety initiatives in accordance with DHS regulations.
• Support staff orientation, competency validation, ongoing education, and required in-service training.
• Assist with completion and monitoring of Resident Assessment Support Plans (RASP), including admission, annual, and significant change assessments.
• Complete DHS Reportable Incident Reports and facility incident documentation.
• Complete annual and significant change Level of Care Assessments.
• Maintain Train-the-Trainer certification for Medication Administration.
• Train, observe, and evaluate Medication Technicians in accordance with state regulations.
• Attend required meetings, training sessions, and continuing education programs.
• Follow all infection prevention, safety, and personal protective equipment (PPE) policies.

• Perform other duties as assigned to support the Personal Care department and community.

Qualifications

• Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
• Supervisory or leadership experience in personal care, assisted living, or long-term care preferred.
• Strong knowledge of medication administration, resident assessments, and DHS regulations.
• Experience with electronic medical records; PointClickCare experience preferred.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
• CPR certification or ability to obtain upon hire.
• Ability to meet all required continuing education and regulatory requirements.

Full Time; 3pm-11pm; e/o weekend and holiday; 80 hours bi-weekly; salaried position
