Copa Health is seeking a ACT Rehabilitation Specialist to join out Assertive Community Treatment team. The ACT Rehab specialist is responsible for helping members obtain and maintain involvement in employment and/or community-based activities. Works as member of a clinical team to assess member needs, participates in the development of individual service plans, and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the ACT team’s specialist for rehabilitation services by collaborating with other team members in the assessing and delivery of rehabilitation skills training and meaningful community activity as specified in the individual consumer’s service plan.

Copa Health is seeking a ACT Rehabilitation Specialist to join out Assertive Community Treatment team. The ACT Rehab specialist is responsible for helping members obtain and maintain involvement in employment and/or community-based activities. Works as member of a clinical team to assess member needs, participates in the development of individual service plans, and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the ACT team’s specialist for rehabilitation services by collaborating with other team members in the assessing and delivery of rehabilitation skills training and meaningful community activity as specified in the individual consumer’s service plan.
Key Responsibilities
- Works as part of a clinical team to facilitate recovery and inspire hope for members while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan.
- Meets with members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.
- Works with member’s Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan.
- Coordinates with all parties involved in member’s treatment according to policy and procedure and clinical protocols, including but not limited to attending inter and intra-agency staffing, communication by telephone and letter, service/support delivery, etc., as indicated on the member’s r’s service plan.
- Assists members and families in accessing additional needed services and obtaining entitlements, including identification of funding services.
- Responsible for transporting members to necessary services, as outlined in their service plan, as needed.
- HS Diploma/GED and six (6) years of full-time direct care experience.
- Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with two (2) years experience preferred.
- Must have one (1) year of training or experience in vocational rehabilitation and support. Prior experience may include job development, job coaching, or supported employment highly preferred.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
- TB Test clearance required at the time of hire.
