Primary Care Physician for Sickle Cell Medical Home

This role is for a board-certified or eligible Family Medicine or Internal Medicine physician with a focus on managing patients with sickle cell disease.

Responsibilities Include

• Providing outpatient care and pain management for adult sickle cell patients
• Collaborating with a multidisciplinary team including social workers, pharmacists, nurses, and wellness coaches
• Working alongside a board-certified hematologist Medical Director and advanced practice providers
• Accessing specialized outpatient hematology clinics, infusion services, and transfusion resources
• Participating in a program active in national sickle cell initiatives and partnerships
  
  

Qualifications And Skills

• Board certification or eligibility in Family or Internal Medicine
• Strong interest in sickle cell disease management and outpatient pain control
• Ability to collaborate within a multidisciplinary team in a comprehensive care setting
• Excellent communication and teamwork skills
