- Deliver exceptional primary care and conduct routine and acute office visits.
- Administer wellness visits, health assessments, and coordinate care with other providers and specialists.
- Lead population health initiatives, ensure adherence to screening guidelines, and assist with patient outreach and education.
- Participate in home visits and promotional activities as needed.
- Master's in Nursing, DNP, or NP certification with active state license and DEA registration.
- National certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties.
- Experience in primary care, understanding of HCC documentation, ICD-10 coding, and health risk assessments (preferred).
- Strong teamwork, communication skills, and a passion for population health.
