Position Title: Outreach Case Manager
Reports To: Program Manager / Program Director
FLSA Status: Full-Time, Non-Exempt
Hours: 35 hours per week
Schedule: Includes one evening shift per week and two (2) Saturdays per month
Location: Hartford, CT (community-based position with regional travel)
Hourly Rate: $21.00 – $25.00 per hour (commensurate with experience)
Position Summary
The Outreach Case Manager plays a critical role in advancing CTHRA’s mission to reduce harm, promote wellness, and improve the quality of life for individuals affected by substance use, homelessness, and systemic inequities. This position provides community-based outreach, harm reduction services, and case management support, connecting participants to vital resources such as healthcare, housing, behavioral health, and social services. The ideal candidate will be fluent in both English and Spanish, possess strong interpersonal and cultural competency skills, and demonstrate an unwavering commitment to equity, inclusion, and trauma-informed care. This role requires flexibility, empathy, and the ability to engage meaningfully with people from diverse backgrounds and lived experiences.
Key Responsibilities
- Conduct intake and comprehensive needs assessments to identify participant goals and barriers.
- Develop individualized service plans that empower participants to define and pursue their own priorities.
- Provide consistent case management, referrals, and follow-up to ensure linkage to housing, healthcare, treatment, and social supports.
- Distribute harm reduction supplies (naloxone, safer use kits, condoms, fentanyl test strips) and provide education on overdose prevention and safer drug use practices.
- Maintain a participant-centered approach rooted in harm reduction, dignity, and nonjudgment.
- Conduct regular street-based outreach, including mobile unit operations, encampment visits, and community-based engagement across Hartford County and surrounding areas.
- Establish and maintain trust with participants through culturally responsive, trauma-informed communication.
- Provide bilingual (English/Spanish) support during outreach to ensure accessibility for Spanish-speaking community members.
- Represent CTHRA in the community, promoting inclusive and stigma-free access to services.
- Work one (1) evening shift each week to increase accessibility for participants outside of standard business hours.
- Work two (2) Saturdays each month as part of the agency’s rotating outreach schedule.
- Maintain flexibility to respond to outreach needs during community events, emergency responses, or high-need periods.
- Accurately document all participant interactions, referrals, and outcomes in agency databases.
- Track service delivery metrics for program evaluation and grant reporting purposes.
- Ensure compliance with confidentiality standards under HIPAA, 42 CFR Part 2, and agency policy.
- Work closely with Program Managers, Outreach Specialists, Peer Navigators, and partner agencies to coordinate wraparound supports.
- Participate in multidisciplinary case conferences and team meetings.
- Advocate for participants across health, housing, and legal systems to reduce barriers to care.
- Attend mandatory agency trainings on harm reduction, trauma-informed care, motivational interviewing, and racial equity.
- Engage in ongoing professional development and reflective supervision to strengthen skills and prevent burnout.
- Contribute to a supportive team culture grounded in transparency, accountability, and compassion.
Qualifications
Required:
- Fluent in both English and Spanish (spoken and written). This is a required qualification.
- Bachelor’s degree in Social Work, Human Services, Public Health, or related field or equivalent lived/professional experience.
- Minimum of two (2) years of experience providing outreach, case management, or direct services to individuals who use drugs, are unhoused, or face systemic barriers.
- Demonstrated understanding of harm reduction philosophy, public health principles, and trauma-informed engagement.
- Strong organizational, communication, and boundary-setting skills.
- Valid Connecticut driver’s license and reliable transportation.
- Ability to work evenings and Saturdays as required.
Preferred:
- Lived experience with substance use, recovery, homelessness, or sex work.
- Familiarity with local service systems (housing, healthcare, behavioral health, criminal justice).
- Proficiency in using databases or case management software.
Working Conditions
- Work is performed in both office and field environments, including street outreach and community events.
- Regular exposure to outdoor elements, varying weather, and community-based settings.
- Must be comfortable engaging individuals in active substance use or experiencing housing instability.
- Use of agency vehicle or personal vehicle (mileage reimbursed) required for outreach.
