What are the responsibilities and job description for the Social Worker (Skilled Nursing Facility) position at Confidential?
Full-time Social Worker for a skilled nursing and long-term care community in northwest Wisconsin.
You'll know your residents here. The census is small, the team is solid, and you won't be buried under a caseload you can't manage.

The work: psychosocial assessments and care plans, families and residents as your main relationships, discharge planning with nursing and therapy, advance directives and referrals, care conferences and MDS.
What we need: a degree in social work, human services, psychology, or something close, plus Wisconsin certification or the willingness to get it. New grads with a good placement, come talk to us.
Benefits: health, dental, and vision. Paid time off. Referral bonus. Weekday schedule.
