What are the responsibilities and job description for the Social Worker position at Ebenezer?
We are looking for a Social Worker to support and advocate for residents in coping with the psychosocial aspects of their lives, including grief, loss, and change. To coordinate transitions, including room changes, discharge plans, and end-of-life preferences.
- 80 hours every two weeks

We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. Responsibilities:
- Manage resident caseloads
- Coordinates admissions with the admission team assigned
- Serve as Resident Advocate and Facility Liaison for Family Council as appropriate
- Facilitates necessary discharge planning and coordination
- Assists in Vulnerable Adult investigation and completes necessary documentation and reports
- Perform other duties as assigned and directed
Required:
- Bachelor's Degree: List – BSW (from an accredited school of social work)
- Licensed by the State of Minnesota Board of Social Work
- 0-1 years Experience
Preferred:
- Two years’ experience in LTC or TCU
- Experience in Medical Social Work
- 1-3 years of experience
